Unilateral recurrent otitis media with effusion caused by a giant petrous internal carotid artery aneurysm: Case report and review of literature

Abstract
We present a case of unilateral recurrent otitis media with effusion and eustachian tube dysfunction caused by a giant aneurysm arising from the petrous segment of the internal carotid artery (ICA). Petrous ICA aneurysms are rare and can present with a variety of signs and symptoms, including pulsatile tinnitus, epistaxis, otorrhagia, cranial nerve palsies, Horner¡¯s syndrome, and middle ear effusion. However, unilateral recurrent middle ear effusion might be presented as the only clinical feature in petrous ICA aneurysms, which necessitates early radiologic studies for accurate diagnosis. Treatment options of petrous ICA aneurysms include close observation, endovascular therapies and surgical trapping with or without revascularization. The authors report one case of unilateral recurrent otitis media caused by a giant petrous internal carotid artery aneurysm including review of literature.
